Does a High PSA Always Mean Cancer? When is a Fusion Biopsy Necessary?

As Prof. Dr. Aykut Recep Aktaş, in my specialty of Interventional Radiology, I frequently observe patients' concerns regarding prostate health. A rise in PSA (Prostate-Specific Antigen) levels, in particular, can lead to fear of prostate cancer in many men. However, a high PSA does not always signify cancer, and understanding the reasons behind this is vital for charting the correct diagnostic and treatment pathway.

What is PSA and Why Does It Rise?

PSA is a protein produced in the prostate gland, ensuring the fluidity of seminal fluid. Its presence in the blood at a certain level is normal. Yet, an elevation above age-dependent reference ranges can, in some cases, indicate a risk of prostate cancer, while often being linked to non-cancerous causes. It is crucial to understand that a high PSA alone is not a diagnostic marker but rather a warning signal.

Non-Cancerous Causes of High PSA

Many non-cancerous conditions can cause PSA levels to rise. Awareness of these conditions can reduce unnecessary anxiety and help determine the appropriate diagnostic approach:

Benign Prostatic Hyperplasia (BPH - Enlarged Prostate): A benign enlargement of the prostate gland commonly seen in a large proportion of men with age. The enlarged prostate tissue produces more PSA, elevating blood levels.

Prostatitis (Prostate Inflammation/Infection): In cases of prostate gland inflammation, PSA levels can increase significantly. This condition often presents with symptoms like burning during urination and frequent urination, and PSA levels can normalise once controlled with antibiotic treatment.

Urinary Tract Infections (UTIs): Although less common, infections in surrounding tissues can also affect PSA.

Recent Procedures: PSA can temporarily rise after urological procedures such as prostate massage, catheter insertion, cystoscopy, or even recent sexual activity (ejaculation). Therefore, it is important to consider such factors before a PSA measurement.

When Does Cancer Suspicion Increase, and Further Investigation Become Necessary?

A PSA value is more than just a number. Factors such as age, previous PSA values, the rate of PSA increase (PSA velocity), and the free PSA/total PSA ratio play a critical role in assessing the risk of prostate cancer. Specifically, a low free PSA ratio can heighten cancer risk. Your urology specialist will evaluate these factors comprehensively to determine the need for further investigation.

MRI and Robotic Fusion Biopsy: The Gold Standard

Traditionally, in suspicious cases, random (blind) transrectal ultrasound (TRUS)-guided biopsies were performed. However, this method had significant disadvantages: the risk of missing cancerous tissue, particularly aggressive cancers, and a high number of unnecessary biopsies. Today, revolutionary advancements have been made in this field.

The Role of Multiparametric MRI

Prostate multiparametric MRI (mpMRI) has become the gold standard for patients with suspected prostate cancer. mpMRI provides detailed imaging of the prostate gland, allowing for high-accuracy identification of potential cancerous foci. PI-RADS scoring is used in these images to grade the cancer risk of lesions. As a radiologist, my role is to meticulously analyse these images, identify suspicious areas, and map out a biopsy roadmap.

Fusion Biopsy: Targeted and Definitive Diagnosis

In cases of high PSA and especially when a suspicious lesion is detected on mpMRI, the fusion biopsy method comes into play. Fusion biopsy works by overlaying (fusing) previously acquired prostate MRI images with real-time ultrasound images. This enables precise targeting of suspicious regions identified on the MRI screen during the ultrasound.

High Accuracy: By directly focusing on suspicious lesions, fusion biopsy significantly increases the detection rate of clinically significant cancers.

Fewer Unnecessary Biopsies: As it is a targeted approach, the number of unnecessary and blind samples taken is reduced. This enhances patient comfort and lowers the risk of complications.

Robotic Fusion Biopsy: With technological advancements, robotic fusion biopsy systems have also evolved. These systems enable ultra-precise guidance of the biopsy needle to the target lesion. Thanks to the stability and precision of robotic arms, the margin of error is minimised, and diagnostic accuracy reaches its peak. As an Interventional Radiologist, this technology offers me and my team the capability to provide the most definitive diagnosis for our patients.

A high PSA is certainly not a cause for panic; however, it is an important signal that should not be ignored. Early diagnosis is life-saving in prostate cancer.
